CONFIRE’s Relationship With Priority Ambulance
Priority Ambulance, the nation’s second-largest ambulance provider, will assist CONFIRE with the management of the ambulance transportation for a large portion of San Bernardino County. CONFIRE retained Priority Ambulance through a formal Request for Proposal (RFP) process to begin a public-private alliance model. CONFIRE’s commitment to excellence in service and stewardship has led us to a public-private partnership. This partnership ensures that we combine our commitment to serve the community with the efficiency of private business practices. Our collective efforts enable us to allocate emergency ambulances to meet the needs and demands of a growing county.

How is CONFIRE preparing for October 1?
CONFIRE has spent years preparing for implementation through extensive coordination with hospitals, fire agencies, law enforcement, labor organizations, dispatch personnel, and community partners. Training, ambulance deployment planning, communications, technology integration, and stakeholder outreach are all underway to ensure a seamless transition and uninterrupted service beginning October 1.
What is CONFIRE EMS and why was it created?
CONFIRE EMS was established to provide coordinated emergency medical services throughout San Bernardino County, including 9-1-1 ambulance response, interfacility transport, and critical care transportation. By integrating dispatch, communications, and ambulance operations, CONFIRE helps ensure patients receive the right care, at the right time, with the right resources. The regional system is designed to improve coordination, strengthen resource deployment, and enhance patient care across the county.
Who is part of CONFIRE EMS?
CONFIRE EMS is a partnership of regional fire agencies serving communities throughout San Bernardino County, including Rancho Cucamonga, Ontario, Rialto, Redlands, Chino Valley, Apple Valley, San Bernardino County Fire, and other allied agencies.

The partnership enhances ambulance deployment, increases operational flexibility, strengthens accountability, and helps meet the growing demand for emergency medical services throughout San Bernardino County.

More than half of the company’s 610,000 annual patient transports are in response to a public call to 9-1-1, while the balance of transports originates through requests from healthcare facilities. Will ambulance service continue without interruption?
Yes. CONFIRE’s highest priority is ensuring residents continue to receive safe, reliable, and high-quality emergency medical services through a seamless transition. Patients should expect uninterrupted ambulance service throughout implementation.
How will CONFIRE improve emergency response?
CONFIRE will coordinate emergency medical resources in real time using advanced dispatch technology, Emergency Medical Dispatching (EMD), and the Emergency Communications Nurse System (ECNS). ECNS connects certain low-acuity callers with registered nurses who help identify the most appropriate level of care, preserving emergency resources for higher-acuity incidents while ensuring patients receive timely assistance.
Will fire departments still respond to medical emergencies?
Yes. Local fire agencies will continue to respond to medical emergencies as part of the emergency response system. CONFIRE EMS is designed to coordinate ambulance transport services with fire, dispatch, hospitals, and other public safety partners.
Will residents notice a change when they call 9-1-1?
No. Residents should continue calling 9-1-1 as they do today. CONFIRE will continue serving as the regional fire/EMS dispatch agency and secondary PSAP. The improvement will occur behind the scenes. CONFIRE will directly dispatch and coordinate ambulance resources, helping reduce handoffs and improve coordination between fire, EMS, and ambulance response.
Will patients continue to be transported to the appropriate hospital?
Yes. Patients will continue to be transported to the most appropriate hospital based on their medical condition, EMS protocols, specialty care availability, and destination policies.
How do we know there will be enough ambulances for our city and ensure they are not all deployed elsewhere in the county?
CONFIRE will have real-time 24/7, 365 days a year, systems and ambulance coordinators in our Command Center who will be responsible for deploying and monitoring ambulances throughout the Comprehensive Service Area. CONFIRE partnered with world-class leaders in the ambulance transportation industry to provide complex ambulance demand analysis and deployment models that best serve our communities.
Will billing or insurance change?
Billing processes may change as CONFIRE EMS and Priority Ambulance assume responsibility for ambulance transport services. Medicare, Medi-Cal, and private insurance will continue to be accepted in accordance with applicable federal, state, and local requirements. Additional information regarding billing and customer service will be provided as implementation progresses.
What will happen to current AMR employees?
CONFIRE and Priority Ambulance value the incumbent workforce and intend to hire as many AMR employees as possible. Priority Ambulance has committed to honoring applicable labor agreements and recognizing employee tenure for those who transition to the new organization.
